I would agree, I think it's just lag.  You probably see the guy right in front of you but he may have moved to the side before you shot.  I get this sometimes.  It's frustrating but it happens.

I've noticed that too! Sometimes it seems to me as if it's an internet connection problem. I
can't seem to get an internet service provider that works well with Xbox.

Had Charter and I got disconnected from the internet in the middle of games!

Now we have a local one and it doesn't ever do stuff like that, but every now and then our
green lights will go red and we'll get knocked out of a game. Plus there's lagging sometimes
and that dreaded "Your Nat Is Not Open" crap that is impossible to fix it seems.
